Is the Texas Department of Licensing and Registration asking for a statement that you choose not to get workers' compensation insurance pursuant to Subchapter A, Chapter 406, Labor Code?
Are you a Texas employer or an employer with employees based in Texas?
Are you a governmental entity?
Do you currently have workers' compensation coverage for your employees at all your locations?
Do you have employees that have declined workers' compensation coverage?

You are required to file a notice of no Texas workers' compensation coverage. Request access to an E-File account here.

All Texas governmental entities are required to have coverage. If you have questions, email coverage.verification@tdi.texas.gov or call (512) 804-4345, Monday to Friday, 8 a.m. to 5 p.m., Central time.

You are required to report work-related injuries and illnesses for employees that have declined your workers' compensation insurance. Request access to an E-File account here.

You are not required to file this notice.
